CHS PARTICIPANT SERVICES

Participant Services are available to CHS’ Housing, Shelter, and Residential program participants only.


HELP AT HOME

Help at Home provides a variety of services to help support individuals living with a disability while remaining in their homes. Services include light housekeeping, meal preparation, running errands, motivation and instruction, and practicing strategies to reduce social isolation. 

PEER SUPPORT SERVICES

Peer Support Services assign a Peer Support Specialist to assist participants in several areas including life skills, service and benefits navigation, substance use, financial literacy, advocacy, and job search and preparation.

Peer Support Specialists add a level of support and care to traditional case management, allowing our participants to accelerate their path toward independence.  

THERAPEUTIC SERVICES

Therapeutic Services provide participants with a variety of nursing services and access to a mental health clinic.

CHS is staffed with a Licensed Social Worker and Certified Registered Nurse Practitioner, allowing participants access to services upon referral.
