Residents enrolled in this permanent housing program at Wood Street Commons receive assistance with room and board, care management, supportive counseling, daily life skills training, medication monitoring, and referral to local resources. The program also creates opportunities for celebration, socialization and skills building.
Work Toward Sustainability From Crisis at Wood Street Commons
The Work Toward Sustainability from Crisis program assists homeless disabled individuals enter permanent housing at Wood Street Commons, create a personal goal plan and receive ongoing support to achieve those goals.
Who We Serve:
Work Toward Sustainability from Crisis aids individuals living with a disability who are experiencing homelessness. Twenty single room occupancy units are available through this program.
This program provides a safe and affordable housing option with supports that allows individuals to receive support and work toward personal goals. This results in individuals increasing their autonomy and feelings of self-esteem, enhancing their income, making successful connections with the local service system and lowering their use of emergency services.
CHS introduced this program in 2010 to respond to the growing number of individuals requesting permanent housing options.
