The Director of In-Home Services is responsible for the management, development, leadership, and vision of home-based health programs. These services include in-home supports and personal care to adults and seniors living with disabilities as well as services to support people with a hoarding behaviors. All of these programs share a focus on building on participants’ strengths to live independently in their homes, achieving health and wellness outcomes, and fostering their independence and integration into the local community.
The Director is responsible for managing the functions of the team, including supervising all employees covering all day-to-day responsibilities of the team. If necessary, the Director will hire and train new members of the team.
The Director ensures services are performed in accordance with program, funder, and agency regulations as well as the individualized service plans for each participant. The Director serves as a primary contact with funders including HSDF, OBH, the United Way and the Area Agency on Aging, ensuring the program remains in compliance and all required reports are completed. The Director ensures consistency and quality of services throughout the program. The Director is responsible for making sure the program is financially sustainable, including developing new funding opportunities as appropriate in conjunction with agency leadership.
- Administers, supervises, and oversees all aspects of the in-home programs, including the hoarding pilot program.
- Reviews all new applications and assists the intake team and Administrative Coordinator with eligibility determination for participants.
- Reviews weekly scheduling of In-Home Specialists and participants with the Administrative Coordinator to ensure efficient operations.
- Ensures all staff and participants receive periodic supervised home visits, re-assessments, and revised service plans.
- Ensures all participants are engaged in the right level of service, including making referrals to the Department of Aging for each participant aging out of services and referrals to other providers for different levels of care or supplemental services.
- Identifies and develops new program and funding opportunities for in-home services, including preparing proposals for funding.
- Lead the team to provide excellent customer service and achieve customer satisfaction levels over 90%, including managing and correcting any negative customer experiences.
- Provide direct supervision to the employees on the in-home team in accordance with all CHS policies, including holding regular team meetings and one-on-one supervision meetings, completing performance development plans, and any hiring and training needed.
- Coach and train staff on assessment and engagement with participants focusing on physical and behavioral health goals as well as improved ability to perform activities of daily living.
- Provide financial oversight of the programs, ensuring that they operated efficiently and on a balanced budget.
- Ensure that the team achieves high quality data standards for all customers served, including the CHS database and SAMS.
- Ensure compliance with all agency policies and procedures including all administrative, human resource and fiscal responsibilities.
- Ensure compliance with all program requirements to DHS, AAA, United Way, and any other funders, including all program monitoring visits and reports.
- Serve as the primary point of contact with all funders and community partners for the in-home services programs.
- BA/BS degree or any combination of life, work and educational experiences.
- 4 years of experience working in physical or behavioral health services.
- 2 years of supervisory experience and demonstrated supervisory skills.
- Experience working in the physical and/or behavioral health system and a working knowledge of the county social service system.
- Strong foundation in mental health diagnoses, including the ability to assess and develop service plans and coach staff on strategies to work effectively with people living with multiple diagnoses.
- Strong crisis management, organization, communication, supervisory, and customer service skills.
- Ability to manage multiple components of a project in various stages of completion.
- Sensitivity toward individuals and families in need of program services, including a strong foundation in harm reduction and trauma informed care.
- Advanced computer literacy.
- The ability to travel independently.
