The In-Home Specialist (IHS) is under the direction of the In-Home Program Director and Program Coordinator.  The IHS provides personal care and home making assistance to adults with physical and mental health disabilities. The goal of the program is for the participants to maintain independent living in the community. The IHS services include teaching home care management skills and providing encouragement and support to use these skills.

The Housing Assistance Programs Operations Coordinator will serve as a critical role in supporting the operations of the Housing Assistance Programs, including the Emergency Solutions Grant program.

Oakland Community Food Pantry at CHS
Hours: 
Part Time

The Food Pantry Specialist manages the maintenance and execution of the CHS Food Pantry distributions. They provide logistical support, help manage teams of volunteers, and provide excellent customer service to the pantry customers. The Food Pantry Specialist will be responsible for data entry and management using an internal database to track customers served at pantry.  Position is part-time and will be 20 hours per week.

This position is an entry level position involving basic clerical and accounting skills in the preparation, organization, and processing of financial data and records.  The work requires excellent organization skills and attention to detail in the performance of tasks within prescribed procedures and regulations.  Specific tasks require the use of standard office equipment, including basic computer programs.

The Director of In-Home Services is responsible for the management, development, leadership, and vision of home-based health programs. These services include in-home supports and personal care to adults and seniors living with disabilities as well as services to support people with a hoarding behaviors. All of these programs share a focus on building on participants’ strengths to live independently in their homes, achieving health and wellness outcomes, and fostering their independence and integration into the local community.

The Intake Coordinator Level 2 (IC2) will work in conjunction with the Director of Customer Service and Intake to ensure efficient assessment and program assignment procedures for CHS programs.

The Intake Coordinator Level I (IC1) position is the first resource connection for families and individuals seeking services from CHS. The IC1 will work with potential clients on the phone and in person to understand their situation and determine if the person should be scheduled for intake into CHS or referred to external resources.  If the person is appropriate for CHS services IC1 will connect to appropriate staff members for further assessment.

Immediate need for full time Consumer Liaison at our Wood Street Residential Facility.  The position of Consumer Liaison is under the general supervision of the WSC Program Director.  This position carries no supervisory responsibilities.

The Community Support Specialist (referred to as “CSS”) provides direct care and case management services in a respectful manner to residents in the CHS Residential programs. Key services to be provided include but are not limited to guidance and supportive counseling with life issues, assistance with issues of daily living, housing assistance, encouragement with goal attainment, encouragement with community involvement, and administering prescribed medication.
